West Sussex Mind are looking for a Mental Health Support Coordinator within Rural North Chichester to join the Emotional Wellbeing Service that provide support across Sussex – this is a 6 month fixed term contract.

The successful applicants will provide recovery focused 1:1 sessions, assessments, and risk assessments as a Mental Health Support Coordinator.  The role will also involve managing a caseload, working within a team based across GP surgeries managed by WS Mind and building a rapport with the GP surgeries across their area. This role does involve independently working in the surgeries, in an autonomous way whilst managing a caseload, administrative tasks and other responsibilities.

As a Mental Health Support Coordinator, you will see patients individually, supporting them with their mental health and wellbeing in a goal focused way in up to 6 one-to-one sessions – such as understanding depression, making sense of anxiety, creating Wellness Action Plans etc. You will be able to take a holistic and flexible approach in your work with individuals supporting around both social and psychological issues that arise and have an awareness of other local services and the Pathfinder alliance.
